1. Initial contact is minimum necessary
During first contact and scheduling, please share only the minimum information needed to coordinate a conversation. Detailed records can be discussed after we establish the safest method for document exchange.
2. What not to submit through this site
Do not submit full medical records, complete medication histories, insurance member IDs, Social Security numbers, financial account numbers, or identity document scans through booking notes or unencrypted email.
3. How intake information is used
Inquiry and booking details are used to respond, schedule consultations, determine fit, and support service delivery if engagement proceeds under a written agreement.
4. Retention baseline
If no client relationship is formed, inquiry and booking records are typically deleted within 24 months unless a longer period is required by law or to resolve a dispute.
5. Future contact forms
This website currently prioritizes email and scheduler intake. If a contact form is added later, the same data-minimization boundaries on this page will apply, along with region-appropriate consent controls where required by law.
